IDHR Forms

Complainant Information Sheet (CIS):

If you believe that you have been discriminated against in one of the following areas, you need to fill out the appropriate CIS (form) and mail (or fax) it to IDHR in order to file a charge with IDHR.

Charge Processing Forms

Completed charge processing forms with signatures must be mailed or faxed to IDHR to be considered filed.  Emailed forms will not be processed.

Attorney Packet:
Charge filing forms and instructions for attorneys.

Appearance Form:
To enter the appearance of Complainant’s and/or Respondent’s attorney.

Disabililty Questionnaire:
Information request to a physician regarding a medical condition.

Medical Release and Consent Form:
To consent to the release and use of Complainant’s medical documentation.
(Required for Disability Cases)

Sample Verified Response Form:
A sample format for Respondent to prepare a Verified Response to a charge filed with IDHR.

Voluntary Withdrawal Request Form:
A Complainant should use this form to withdraw a charge of housing discrimination.

Please contact the Department to obtain a Voluntary Withdrawal Request Form for a non-housing charge.  To contact the Department, you can send Email or you can call the Department at (312) 814-4320.  Please be prepared to provide your case information (charge number and party names) when contacting the Department.

File Review Request Form:
You can review a charge file by requesting it in writing.  You must be a party to the charge or have an appearance form on file with IDHR.

Contracts — PC-1 Form

The document includes instructions for completing the PC-1 form.

Completed and signed Employer Report Form (PC-1) must be submitted to the IDHR Fiscal Unit and accompanied by a certified check, money order, or cashier’s check for $75 payable to the Illinois Department of Human Rights.

Faxed or emailed PC-1 Form will not be processed.
