Due to the level of interest expressed, and after allocating space for committee members, presentation guests and required staff, we regret that we cannot confirm in-person attendance for the November 15, 2011, Care Coordination Subcommittee meeting. All confirmations to attend in-person are revoked. Attendance at both the Chicago and Springfield locations will be on a first-come, first-served basis.

This will allow space for those wanting to actively participate, and in particular give input on the performance and quality measures for the Innovations Project, to attend in-person.
