Care Coordination 

Welcome to the Care Coordination Policy Forum. PA96-1501 (pdf) (also known as "Medicaid Reform") requires that 50% of Medicaid clients be enrolled in care coordination programs by 2015. In Illinois, care coordination will be provided to most Medicaid clients by “managed care entities,” a general term that will include Coordinated Care Entities (CCEs), Managed Care Community Networks (MCCNs) and Managed Care Organizations (MCOs).

This site will be the major electronic channel for communication on this policy change. Please continue to consult this site, as we will post information about next steps as available.

Care Coordination Roll-Out Plans

We have released our proposed Care Coordination Roll-Out Plan for the next two years, from January 2013-January 2015. This should be considered a draft work-in-progress but reflects the state’s current thinking on how we will meet the Illinois state law requirement that at least 50% of Medicaid clients will be enrolled in care coordination by January 2015.

On October 13, 2011, HFS held a public meeting to test community interest, and capacity to provide alternative models of delivering care (as an adjunct to our current managed care programs). The purpose of the meeting was to provide the department with feedback on its proposed concepts to use in the planning and development for this project.
