Service We Can Provide
APS is prepared to offer the following services to CCEs- -Assist CCEs in identifying and engaging the costliest, most vulnerable patients -Expertise in health care data analytics and predictive modeling, we identify individuals who have medically-complex conditions that can be impacted by proactive care coordination aimed to improve quality and lower costs. -Leveraging the medical home-We link primary and specialty care, behavioral health care, pharmacy and community services in a coordijnated health care neighborhood. -Local presence-Our locally-based professionals-health coaches, clinicians and peer specialists-work with you to serve patients in their home communities. -Collaborating with providers-We will be able to support the CCE providers by providing them patient information-procedures, adminissions, and prescribed drugs-and we support the treating practitioner by arranging transportation to appointments and providing self-care education. -Deploying Technology-Our web-based, HIPAA compliant CareConnection® platform merges multiple data sources (including Medicare and Medicaid data for dual eligibles) and creates an integrated, interactive electronic health record accessible by all practitioners serving each member.
