Palliative care program to provide in-home symptom management consultations and monitoring for individuals who do not meet hospice criteria but have troublesome symptoms from advanced disease processes, which may be requiring frequent hospitalizations.
Service We Can Provide
In-home nursing and nurse practiioner or physician visits in consulting role with attending physician, or as attending if needed, to decrease hospitalizations and prevent individuals from "falling through the cracks" of the healthcare system. Individuals who meet hospice criteria will also receive full 24/7 benefits of interdisciplinary team home visits from hospice physician, nurse, CNA, social worker, chaplain, therapists, volunteers and bereavement counselor; DME to keep individual safe and comfortable at home, including low-air-loss mattresses to prevent skin breakdown; medications related to terminal illness; and bereavement services for family for one year after the death of their loved one. Data reflecting patient and family satisfaction with healthcare system following referral to hospice program upon discharge from the hospital.
