We are interested in working with the lead organization in our area to provide Home and Commuity Based Waiver Service Facilitation and additional person centered case management that can prevent the need for more costly services.
Additional Information
Our strength is that we are a Family to Family organization. The majority of our staff are relatives of people with disabilities. We know our participants really well and have often served them and their families throughout their life spans. We also know the service providers and services available in the community and have been able to create natural supports around individuals. Our Rep. Payee program is 30 years old and successfully keeps vulnerable people from financial abuse and neglect by ensuring their benefits are used appropriately for shelter, food, medical care as designated by Social Security.
Our goals are:
1.Assist families to raise their children at home.
2.Support adults to live normally in their community through careful use of the services and supports they are entitled to.
3.Keep individuals and their families informed of services and options available to them to enhance their lives in their own homes.
