  Registration can be paid either via U.S. mail or online-

• REGISTRATION & PAYMENT VIA U.S.MAIL:  Fill out the registration form (below) and mail to IDHHC along with a check/money order with the appropriate amount.

• ONLINE REGISTRATION & PAYMENT:  Go to our E-Pay site (see E-PAY shown on left side).  Please take attention to the special conference note shown on E-Pay site.  Please do still look over the registration form (below) to ensure you receive all the information you need for the conference.
