The Benefit Choice Period will be May 1 through June 15, 2012, for all members. The FY2013 Benefit Choice Options booklet contains the latest information regarding the Benefit Choice Period options, as well as other benefit information. Elections will be effective July 1, 2012.
During the annual Benefit Choice Period, members are allowed to make changes to their group insurance coverage, such as add or drop a dependent or change their health plan. Members should make all Benefit Choice changes on the Benefit Choice Election form. Only members wishing to make a change to their coverage should complete the form. The Benefit Choice Election Form must be submitted to your unit Health Plan Representative (HPR) by June 15, 2012.
During the annual Benefit Choice Period, members may:
- Change health plans. Members interested in a certain health plan may view their provider directory on the plan's website.
- Add or drop dependent coverage.

All Benefit Choice changes should be made on the LGHP Benefit Choice Election Form. Members should not complete a form if no changes are being made. Your unit’s Health Plan Representative can answer your benefit questions. Completed forms must be given to the HPR for processing.
