Employees waiting for an Explanation of Benefits (EOB) from either Aetna or Delta Dental in order to file a claim for Flexible Spending Account (FSA) reimbursement from their Medical Care Assistance Plan (MCAP) should read the following:
Quality Care Health Plan (QCHP)
enrolled in the Quality Care Health Plan (QCHP) will not receive an EOB until the claim payment is

employees who need
an itemized statement of their claim should either visit the Aetna
Navigator employee portal and login to print a copy of the itemized statement to send to the FSA
vendor (ConnectYourCare), or call Aetna at 855-339-9731.
Quality Care Dental Plan (QCDP)
Employees who have not yet received an EOB for their dental claim can login to the Delta Dental website and print a copy of their claim in 'FSA View'. This document is not considered an EOB but is accepted by the FSA vendor, ConnectYourCare, as suitable documentation for a dental claim since it indicates the employee's out-of-pocket portion of the claim.
Employees who have not yet received an EOB for their vision claim can login to the EyeMed website and print a copy of their claim/EOB.

Please note that reimbursement requests received after the run-out period (i.e., September 30th) will not be accepted.
