Employees waiting for an Explanation of Benefits (EOB) from either Cigna or Delta Dental in order to file a claim for Flexible Spending Account (FSA) reimbursement from their Medical Care Assistance Plan (MCAP) should read the following:
Quality Care Health Plan (QCHP)
Employees enrolled in the Quality Care Health Plan (QCHP) should receive an EOB within 2 weeks of their doctor's appointment, hospital visit or other healthcare appointment. The hold on healthcare claim payments does not affect the immediate mailing of EOB's from CIGNA. Therefore, employees who have not received an expected EOB within this time frame should either visit Cigna's website and login to print a copy of the EOB to send to the FSA vendor (ConnectYourCare), or call Cigna at (800) 962-0051.
Quality Care Dental Plan (QCDP)
Employees who have not yet received an EOB for their dental claim can login to the Delta Dental website and print a copy of their claim in 'FSA View'. This document is not considered an EOB but is accepted by the FSA vendor, ConnectYourCare, as suitable documentation for a dental claim since it indicates the employee's out-of-pocket portion of the claim.
Employees who have not yet received an EOB for their vision claim can login to the Eyemed website and print a copy of their claim/EOB.

Please note that reimbursement requests received after the run-out period (i.e., September 30th) will not be accepted.
