Disability Forms

 

The online Disability Survey Form is to be completed by each employee of the agency.  The response to the form is voluntary.  The information provided is to be treated confidentially and used for affirmative action statistics and emergency evacuation purposes.  When all the information has been received, the agency will use this data to determine if the agency is at parity or underutilized for people with disabilities.  "LIVE" version of the Disability Survey Form.

The Labor Force Analysis for People with Disabilities (DHR-34-AAP) determines if the agency is underutilized or at parity.  Enter the total number of employees; multiply by the percent of people with disabilities in the Illinois labor force, which is 4.94% (updated annually), and this will indicate the labor force number of people with disabilities you need in your agency workforce.  If the number of employees with disabilities in the agency is greater than your labor force number, then your agency is at parity.  If you have underutilization, then your agency must establish numerical goals for people with disabilities.

The Reasonable Accommodations Form for Employees is a sample form agencies can use for requests for accommodations from employees.  Agencies are free to revise the form as needed.
