Having Trouble Opening These Forms?: These forms must be opened using Acrobat Reader. If you click on one of these links and get a message that starts with "Please wait...", then your computer is trying to use the web browser rather than Acrobat Reader to open the file. In that case, do this:
- Right-click on the link and select "Save link as..." or "Save target as..." to save the file.
- Start Acrobat Reader.
- In Acrobat Reader's menu, select "File", then "Open" to open the saved file.
Combined Demolition/Renovation/Asbestos Project Notification Form: The Combined Notification of Demolition and Renovation form is used for notifications sent to the Illinois EPA, the Illinois Department of Public Health (IDPH), and the City of Chicago. A separate copy must be sent to each agency if you are required to send a form to IDPH and the City of Chicago. Electronic submission to these agencies is not available at this time. Please print, provide original signature, and mail with $150 fee or proof of electronic payment to the address below. (Cook County no longer uses the combined notification form and notifications must be made
via their online system.)
