The Illinois Environmental Protection Agency (IEPA) regulates the transportation of the following wastes: Used Tires, Special Waste including Hazardous Waste, and Potentially Infectious Medical Waste (PIMW). There are no manifest requirements for waste tires, but the other programs require that all loads be accompanied by the appropriate manifest forms purchased from the IEPA.

The Illinois EPA has updated the Illinois Waste Hauling Permit application forms and process to conform to applicable state law. The revisions, which are detailed below, will take effect for applications submitted on or after July 1, 2017.

The Illinois EPA has updated its application forms in three significant respects.

  • First, the Special Waste and Potentially Infectious Medical Waste (PIMW) Hauling Permit applications now include fields used to reflect the date on which a permitted vehicle was first used or permitted to haul special or PIMW waste in Illinois.
  • Second, Special Waste and PIMW Hauling Permit applications that are signed by an owner's or operator's authorized representative must be accompanied by documentation reflecting the scope of activities that the representative is authorized to conduct on the owner's or operator's behalf.
  • Third, Special Waste and PIMW Hauling Permit applicants must include with the application a 39(i) certification that includes, among other necessary information, the disclosure of entity, officer, and employee felony convictions, pending criminal actions, and violations of environmental laws and regulations, if applicable.

Applications not accompanied by completed 39(i) certificates will be rejected.

Transporting Special waste requires:

  1. A special waste hauling permit

  3. To transport RCRA or TSCA waste, you must have notified as a transporter using form 8700-12 and obtained a USEPA ID number.

Transporting Potentially Infectious Medical Waste (PIMW) requires:

  1. A PIMW hauling permit
  2. A PIMW manifest purchased from the IEPA, and
  3. Submission of reports of activities.
