Re:      ​Letters of Intent for Upcoming Healthcare Transformation Collaboratives Funding Due June 15, 2021



This notice is a reminder that entities intending to apply for upcoming Healthcare Transformation Collaboratives funding can still formally notify the Illinois Department of Healthcare and Family Services of their intention.


Entities are strongly encouraged to file a Letter of Intent if they are planning to apply for funding during State Fiscal Year 2022 (July 1, 2021 – June 30, 2022). The Department of Healthcare & Family Services plans to initiate a new round of funding in September 2021.  Letters for those seeking funding during this cycle are to be received no later than June 15, 2021.

About Healthcare Transformation Collaboratives


The Healthcare Transformation Collaboratives program makes available as much as $150 million per fiscal year beginning in State Fiscal Year 2021. This funding is available to collaborations between care providers, including preventative care, primary care specialty care, hospital services, mental health and substance abuse services, and community-based entities that address the social determinants of health. Priority will be given to collaborations that include safety net hospitals or critical access hospitals.


The application period for the first round of awards has closed; however, a second round of applications will be available for the fall.
