Re: Next Round of Healthcare Transformation Collaboratives Funding
This is a reminder that the application form for the next round of Healthcare Transformation Collaboratives funding will be available soon.
The Illinois Department of Healthcare and Family Services is developing a more user-friendly online application form. While the format for submitting applications will change, the application information requested will remain substantially the same. In preparation, entities interested in seeking funds are encouraged to review previous application materials here and register for email alerts to receive notice as soon as the new process begins.
Funding awards will be made during state Fiscal Year 2022.
About Healthcare Transformation Collaboratives
The Healthcare Transformation Collaboratives program makes available as much as $150 million per fiscal year beginning in State Fiscal Year 2021. The goal of this program is to reorient the healthcare delivery system around caring for people's physical health, behavioral health, and social needs in coordinated fashion in local communities.
Funding is available to collaborations between care providers, including preventative care, primary care specialty care, hospital services, mental health and substance abuse services, and community-based entities that address the social determinants of health. Priority is given to collaborations that include safety net hospitals or critical access hospitals.
On June 30, 2021, approximately $90 million was awarded to eight collaboratives as part of the first round of funding. Additional proposals remain under review and more awards may be announced.
