Re: Notice of Supplemental Request for Qualifications for Care Coordination and Support Organizations
This notice informs providers of the posting of a Supplemental Request for Qualifications (RFQ) for Care Coordination and Support Organizations (CCSOs).
The Illinois Department of Healthcare and Family Services (HFS), in collaboration with its partners at the Department of Children and Family Services (DCFS) and Department of Human Services – Division of Mental Health (DHS-DMH) and the HealthChoice Illinois MCOs, issued an initial Request for Qualifications (RFQ) on September 1, 2021, to solicit proposed plans from interested providers to become CCSOs for Designated Service Areas (DSAs) across the State of Illinois.
The Supplemental Request for Qualifications contains revised provider requirements and targets the specific Designated Service Areas within Illinois of 3, 7, 9, 10, 14, 16, 17, 23, 28, 29, and 31.
The Supplemental Request for Qualifications can be found at this link. Interested applicants will have until 5:00pm on Friday, November 5, 2021, to submit applications in response to the Supplemental RFQ. Completed applications are to be submitted via email to HFS.Pathways@illinois.gov. Only applications for the targeted Designated Service Areas of 3, 7, 9, 10, 14, 16, 17, 23, 28, 29, and 31 will be reviewed.
