Re:      Illinois SMHRF and SLP Screening and Assessments: Redesign Implementation with State Partner, Maximus, Planned for April 18, 2022


The State of Illinois, Department of Healthcare and Family Services (HFS) has selected a new partner, Maximus, to support a redesign of the state's Pre-admission Screening and Resident Review (PASRR) system, including new processes for screening and assessments of individuals seeking placement in Specialized Mental Health Rehabilitation Facilities (SMHRF) and Supportive Living Programs (SLP). This transition includes moving the screening and assessment processes to a new web-based management system, AssessmentPro. This critical change will impact SMHRF and SLP screening submissions and completion of assessments, moving forward from the planned transition date.

Implementation Timeline
The transition to AssessmentPro for potential SMHRF and SLP admissions is scheduled
to launch on Monday, April 18, 2022.

Reasons for the Change
HFS made the decision to seek out a new screening and assessment partner for several key reasons, including:

  • Conflict-free screening and assessments 
  • Centralization of SMHRF and SLP screenings, assessments, and reporting in one system, increasing efficiency, timeliness, and ease of use 
  • Immediate availability of screening and assessment outcomes to Stakeholders upon completion 

Changes Planned for Existing SMHRF and SLP Processes
This transition will shift responsibilities and management for screening and assessment processes to the Maximus assessment team. As part of this migration, steps are being taken to better align existing workflows. For those involved in SLP submissions, these changes specifically relate to the Mental Health/Serious Mental Illness (SMI) screening and not the Determination of Need (DON) assessment.

PASRR Redesign and Choices for Care Consultations
Note that Choices for Care consultations will still be completed as a state requirement, but the workflow is being aligned with the redesigned SMHRF and SLP screening processes.

Training Details Coming in February for SMHRF and SLP Stakeholders
Stakeholders who currently have SMHRF and SLP-related responsibilities will be notified via email by Maximus regarding upcoming training opportunities. Sign up for the
