Re: Safety Net Hospital and Federally Qualified Health Center Coordination per Public Act 102-0004
This notice acknowledges the Department's role in facilitating coordination between Federally Qualified Health Centers (FQHCs) and safety-net hospitals as identified in Public Act 102-0004.
The Act requires the Department to develop a program to facilitate coordination between
FQHCs and safety net hospitals, with the goal of increasing care coordination, managing chronic diseases, and addressing the social determinants of health. FQHCs are already able to operate at hospital locations and many do. The Department has encouraged this type of partnership in its Healthcare Transformation Collaborative (HTC) initiative. Several of these FQHC/safety net hospital partnerships were awarded funding in June of 2021 after the HTC initial round of applications.
