Re:      New Agreements, Address Updates, and Application for Benefits Eligibility (ABE) Screen Changes


This notice provides updates to Healthcare and Family Services (HFS) Application Agents regarding new agreement forms, customer address updates, and Application for Benefits Eligibility (ABE) screen changes. 

Reminder on New Agreements for HFS Application Agents

As was communicated via listserv to Application Agents in April, new agreement forms for existing Application Agents are required and should be submitted by July 1, 2022. The Application Agent Agreement, Business Associate Agreement and W-9 Forms can be found on the HFS Application Agent Instructions page.  

  • If you are an existing All Kids Application Agent (AKAA) who no longer wants to participate as an Application Agent Agency, please email  so that HFS can keep its Application Agent Agency list current. 
  • Staff at AKAAs who do not complete the new Agreement will lose access to the Application for Benefits Eligibility (ABE) Partner Portal as of July 31, 2022. Their current agreement will be null and void and they will lose this Category of Service from their provider number. Staff can continue to help applicants set up and submit applications through the applicant's own ABE account. 

Prospective Application Agent Agencies may apply at any time. Information and the Request to Become an Application Agent form can be found on the HFS Application Agent Instructions page. Applications will be reviewed as they are submitted.

Update Addresses

Assist your customers in updating their addresses to help ensure they will receive notices at the end of the public health emergency (PHE). Updating Medicaid addresses is easy, fast and free. Call 877-805-5312 from 7:45am–4:30pm or visit If you use a TTY, call 1-877-204-1012.  HFS encourages use of the Communications Toolkit for standard messaging, fliers, social media, email and texts.  

New ABE Screens

On July 1, 2022, the ABE Application will display additional new questions. After creating an ABE Account, at the beginning of an application, the applicant will need to indicate "Yes" or "No" for each benefit type. This makes for more informed meaningful decisions on the part of the customer. Not answering for any benefit will result in an error message and will prevent the applicant from advancing in the application. Clicking the "More about …" in blue after each benefit listing will bring the user to an informational page about that specific benefit. The questions will be asked again when completing demographic information for each individual on the application.
