Re: Collection of Facility Payee Information for Directed Payments from MCO Plans

Quality Incentive and Certified Nursing Assistant (CNA) Wage Pass-Through payments are being implemented pursuant to Public Act 102-1035 (ilga.gov). These payments will be calculated quarterly and paid on a monthly basis. Payments will be made by Healthcare and Family Services (HFS) as well as by the managed care organization (MCO) plans.
To facilitate the MCO plans being able to process the monthly directed payments through their accounting systems, please:
- Fill out and return the attached Directed Payment Data Collection Form. The form is requesting the necessary facility, contact and banking information for MCO plans to process monthly payments.
- Submit a current W-9, as the MCO plans will be responsible for issuing 1099 forms for these payments.
Directed Payment Data Collection Forms (either Excel or PDF version) and W-9s can be sent to the following email: HFS.LTC@illinois.gov by August 10, 2022. Please note the form name in the email subject line. MCO plans will not be able to distribute Quality Incentive or CNA Wage Pass-Through payments without this payee information.
Once final payment amounts are calculated, HFS will post a detailed listing of each provider's payment including the amounts to be paid by HFS and each MCO plan.
