Re: Pathways to Success Home and Community-Based Services Overview Webinars

The Illinois Department of Healthcare and Family Services (HFS) is launching a new program, Pathways to Success, that will begin serving youth under the age of 21 in January 2023. HFS will be hosting overview webinars regarding the Community-Based Behavioral Health Services available through Pathways to Success.
Pathways to Success includes a care coordination model in conjunction with additional home and community-based services to provide a more robust continuum of intensive behavioral health services for eligible youth.
HFS will host the webinars listed below including an overview of the HCBS services available through Pathways to Success, an overview of Intensive Home-Based Services and an overview of Family Peer Support.
