To register as a provider, you must
Click on the Registration Menu from the left-side navigation bar.
From the MEDI Registration Menu, select Medicaid Provider Registration.
Complete the fields required on the Provider Registration page. The required information can be found on your Provider Information Sheet.
Click Submit.
Tip: If you receive a 'Provider Not Found' error message, check the information you entered. Try using the enrollment date to register rather than the tax ID number.
