The goal of Healthcare Transformation Collaboratives is to reorie​nt o​ur healthcare delivery system in Illinois around people and communities. There are four major components in the Transformation Plan.​​

Focus on community needs, for all levels of healthcare with an emphasis on addressing social and structural determinants of health.

Improve health and wellness for individuals and communities.

We are proud to announce the awardees for HTC's 2021 first round of funding. Governor JB Pritzker signed this historic initiative into law on March 12, 2021. On June 30, 2021, $94.3 million was awarded to the collaboratives below demonstrating innovative visions and equity-focused solutions in support of our goal to reimagine healthcare in Illinois. Additional proposals are under review and more awards will be announced.
