Care Coordination Roll-Out Plan

HFS hosted a webcast on June 30, 2014 on our Illinois Medicaid Care Coordination Roll-Out Plan for almost 2,000 participants.  Please note that the following materials have now been posted on the HFS Website, under the “Care Coordination” tab.  Questions and answers from the webcast will be published next week.

Please note that these documents are primarily for providers and advocates who work with Medicaid clients.  We want to make sure you are informed of this process every step of the way.  We encourage you to host educational programs for practitioners within your facility.  For a Medicaid provider, it’s important that you understand that your patients are or will be enrolled with a Health Plan offered by a managed care entity if they live in one of the five mandatory managed care regions. Medicaid clients will no longer be able go “anywhere that accepts Medicaid.”  You can take steps to keep your patients under your care.  Please work with the Health Plans to join their networks and then become familiar with the providers and services within the network.  If you have questions for the Illinois Medicaid Program, please reach us at  Thank you for your ongoing support and involvement.
