Retailer Application.pdfRetailer Application
The cost of a Retail liquor license is $750.00. You must have your local liquor license, Sales Tax number /Illinois Business Tax (IBT) number, and Federal Employer Identification Number (FEIN) before you can receive your state retail liquor license. A certificate of insurance is required for licensees with on-premise consumption or a combination of on/off-premise consumption. You must also provide a photocopy of your local license. NOTE: A receipt from the purchase of your local license is not acceptable.
Special Use Permit.pdfSpecial Use Permit
A Special Use Permit is required for current retail liquor license holders to hold any special off-premise event(s). The cost of the license is $150.00 for a one day event, $250.00 if the event's duration is two days or more up to a maximum of 15 consecutive calendar days. There is a $25 late fee if the license is applied for less than 14 days before the event. Local approval and Dram Shop insurance are required.

A Specialty Retailer license includes the following license classes and fees: Brew Pub—$1,500.00; Caterer Retailer—$500.00; Wine Maker Retailer—$500.00; and Wine Maker Retailer (Second Location)—$1,000.00. The application includes instructions and lists all supporting documents necessary to obtain the license.
Homebrewer Special Event Permit.pdfHomebrewer Special Event Permit
The cost of this license is $25. A homebrewer special event permit is required when a person offers homemade brewed beverages samples to the public at any location.
Distributor Application.pdfDistributor Application
The cost of the license varies per gallons. If you import product from both out-of-state and out-of-country, you will have to pay the initial Distributor Fee plus $50. REQUIRED: You must provide the Liquor Control Commission with a copy of your Federal Basic Permit (http://www.ttb.gov/forms/f510024.pdf), a warehouse address, and one of the following Illinois Dept. of Revenue Bond Forms: REG-4-A (Financial Responsibility Bond); or REG-4-D (Financial Institution Irrevocable Letter of Credit Bond).

A winery must have this license prior to directly shipping a maximum of 12 cases of wine per adult Illinois resident per year. The cost of the license is $350.00 for wineries, if Self Distribution is added the cost lof the license is $1,500.00 manufacturing under 250,000 gallons annually; $1,000.00 if manufacturing 500,000 gallons or less a year; and $1,500.00 if manufacturing over 500,000 gallons a year. REQUIRED: You must provide the Liquor Control Commission with a copy of all Federal Label/Bottle Approvals (http://www.ttb.gov/forms/f510031.pdf) and one of the following Illinois Dept. of Revenue Bond Forms:  REG-4-A (Financial Responsibility Bond); or REG-4-D (Financial Institution Irrevocable Letter of Credit Bond).

The cost of a Broker's License is $1,000.00. A broker's license is required for any person who solicits orders for, offers to sell or offers to supply alcoholic liquor to retailers in the State of Illinois, or who offers to retailers to ship or cause to be shipped or to make contact with distillers, rectifiers, brewers or manufacturers or any other party within or without the State of Illinois in order that alcoholic liquors be shipped to a distributor, importing distributor or foreign importer, whether such solicitation or offer is consummated within or without the State of Illinois.
Non-Beverage User Application.pdfNon-Beverage User Application
A Non-Beverage User's license allows for the purchase of alcoholic liquor from a licensed manufacturer or importing distributor—without the imposition of any tax upon the business of such licensed manufacturer or importing distributor. The alcoholic liquor can be used solely for the non-beverage purposes set forth in Section 8-1 of the Liquor Control Act. A Non-Beverage User's license consists of the following license classifications: Class 1—not to exceed 500 gallons; Class 2—not to exceed 1,000 gallons; Class 3—not to exceed 5,000 gallons; Class 4—not to exceed 10,000 gallons; and Class 5—not to exceed 50,000 gallons.

The cost of this license is $300.00.  A Tasting Representative License allows for the supervised presentation of alcoholic beverages off-premises for the purpose of disseminating product information and education.
BASSET Application.pdfBASSET Application
The Beverage Alcohol Sellers and Servers Education and Training (BASSET) license is $350.00 annually. You must submit a copy of your training curriculum, the pre- and post-tests, a copy of the certificate of course completion, along with your Sales Tax/ Illinois Business Tax (IBT) number, and Federal Employer Identification Number (FEIN) in order to receive your BASSET license.
Craft Distiller Tasting.pdfCraft Distiller Tasting
The fee for a craft distiller tasting permit license is $25. A Craft Distiller Tasting Permit Licensee may conduct product sampling of distilled spirits transferred from the craft distiller licensed premises for consumption at the location specified in the Craft Distiller Tasting Permit License. Up to three samples, consisting of no more than 1/4 ounce of distilled spirits, may be served to a consumer in one day. The licensee is prohibited from selling or reselling the transferred liquor. The applicant is required to provide proof of dram shop liability insurance to the maximum limits and have local authority approval. 
Special Event Retailer Liquor License NFP.pdfSpecial Event Retailer Liquor License NFP
The cost of a Special Event Retailer license is $25. There is a $25 late fee if the license is applied for less than 14 days before the event. You must prove Not-for-Profit status and provide a physical address for the location for the event. You may not use a P.O. Box or a Rural Route for the organization's address (a P.O. Box can be used for the mailing address only). Local approval and Dram Shop insurance are required.

A brewer warehouse permit may be issued to the holder of a class 1 brewer license or a class 2 brewer license. If the holder of the permit is a class 1 brewer licensee, the brewer warehouse permit shall allow the holder to store or warehouse up to 930,000 gallons of tax-determined beer manufactured by the holder of the permit at the premises specified on the permit. If the holder of the permit is a class 2 Brewer licensee, the brewer warehouse permit shall allow the holder to store or warehouse up to 3,720,000 gallons of tax-determined beer manufactured by the holder of the permit at the premises specified on the permit. Sales to non-licensees are prohibited at the premises specified in the brewer warehouse permit.
Brewer's Class 1-2 License Application.pdfBrewer's Class 1-2 License Application
Class 1 Brewer License: The cost of this license is $75.00. A class 1 brewer is a licensed brewer or licensed non-resident dealer who manufactures not more than 930,000 gallons (30,000 barrels) of beer per year. A class 1 brewer may make sales and deliveries to distributors and importing distributors, and can apply for a self-distribution exemption allowing for the sale of not more than 232,500 gallons (7,500 barrels) of the exemption holder’s beer to retailers annually, but may not hold a brew pub license. Class 2 Brewer's License: The cost of this license is $100.00. A class 2 brewer is a licensed brewer or licensed non-resident dealer who manufactures up to 3,720,000 gallons (120,000 barrels) of beer per year. A class 2 brewer may make sales and deliveries to distributors and importing distributors, but is not eligible for a self-distribution exemption. A class 2 brewer may hold up to three brew pub licenses, and annually transfer up to 3,720,000 gallons (120,000 barrels) of manufactured beer between commonly-owned class 2 brewer locations.

A craft distiller warehouse permit may be issued to the holder of a class 1 craft distiller or class 2 craft distiller license. The craft distiller warehouse permit shall allow the holder to store or warehouse up to 500,000 gallons of spirits manufactured by the holder of the permit at the premises specified on the permit.
Sales to non-licensees are prohibited at the premises specified in the craft distiller warehouse permit.

A Non-resident Dealer’s License shall permit such licensee to ship into and warehouse alcoholic liquor in this state from any point outside of this state, and to sell such alcoholic liquor to Illinois-licensed foreign importers and importing distributors and to noone else in this state; provided that said non-resident dealer shall register with the Illinois Liquor Control Commission each and every brand of alcoholic liquor which it proposes to sell to Illinois licensees during the license period; and further provided that it shall comply with all of the provisions of Section 5/6-9 of the Illinois Liquor Control Act with respect to registration of such Illinois licensees as may be granted the right to sell such brands at wholesale. Please note that you must appoint an Illinois-licensed distributor (see page 3, item 10). REQUIRED: In addition to the above application and its supporting files, you must provide the Illinois Liquor Control Commission with copies of your Federal Basic Permit (http://www.ttb.gov/forms/f510024.pdf) and all Federal Label/Bottle Approvals (http://www.ttb.gov/forms/f510031.pdf).
Class 1-3 Brewer Application.pdfClass 1-3 Brewer Application
A class 1 brewer license, which may only be issued to a licensed brewer or licensed non-resident dealer, shall allow the manufacture of up to 930,000 gallons of beer per year provided that the class 1 brewer licensee does not manufacture more than a combined 930,000 gallons of beer per year and is not a member of or affiliated with, directly or indirectly, a manufacturer that produces more than 930,000 gallons of beer per year. If a class 1 brewer manufactures spirits, it shall also obtain and shall only be eligible for, in addition to any current license, a class 1 craft distiller license, shall not manufacture more than 50,000 gallons of spirits per year, and shall not be a member of or affiliated with, directly or indirectly, a manufacturer that produces more than 50,000 gallons of spirits per year. If a class 1 craft brewer manufactures wine, it shall also obtain and shall only be eligible for, in addition to any current license, a first-class wine-manufacturer license or a first-class wine-maker's license, shall not manufacture more than 50,000 gallons of wine per year, and shall not be a member of or affiliated with, directly or indirectly, a manufacturer that produces more than 50,000 gallons of wine per year. A class 1 brewer licensee may make sales and deliveries to importing distributors and distributors and to retail licensees in accordance with the conditions set forth in paragraph (18) subsection (a) of Section 3-12 of this Act. If the State Commission provides prior approval, a class 1 brewer may annually transfer up to 930,000 gallons of beer manufactured by that class 1 brewer to the premises of a licensed class 1 brewer wholly owned and operated by the same licensee.

If a class 1 craft distiller manufactures beer, it shall also obtain and shall only be eligible for, in addition to any current license, a class 1 brewer license, shall not manufacture more than 930,000 gallons of beer per year, and shall not be a member of or affiliated with, directly or indirectly, a manufacturer that produces more than 930,000 gallons of beer per year. If a class 1 craft distiller manufactures wine, it shall also obtain and shall only be eligible for, in addition to any current license, a first-class wine-manufacturer license or a first-class wine-maker's license, shall not manufacture more than 50,000 gallons of wine per year, and shall not be a member of or affiliated with, directly or indirectly, a manufacturer that produces more than 50,000 gallons of wine per year.A class 1 craft distiller licensee may make sales and deliveries to importing distributors and distributors and to retail licensees in accordance with the conditions set forth in paragraph (19) of subsection (a) of Section 3-12 of this Act. However, the aggregate amount of spirits sold to non-licensees and sold or delivered to retail licensees may not exceed 5,000 gallons per year. A class 1 craft distiller licensee may sell up to 5,000 gallons of such spirits to non-licensees to the extent permitted by any exemption approved by the State Commission pursuant to Section 6-4 of this Act. A class 1 craft distiller license holder may store such spirits at a non-contiguous licensed location, but at no time shall a class 1 craft distiller license holder directly or indirectly produce in the aggregate more than 50,000 gallons of spirits per year. A class 1 craft distiller licensee may hold more than one class 1 craft distiller's license. However, a class 1 craft distiller that holds more than one class 1 craft distiller license shall not manufacture, in the aggregate, more than 50,000 gallons of spirits by distillation per year and shall not sell, in the aggregate, more than 5,000 gallons of such spirits to non-licensees in accordance with an exemption approved by the State Commission pursuant to Section 6-4 of this Act. A beer showcase permit means a license for use by a class 3 brewer or distributor to allow for the transfer of beer only from an existing licensed premises of a class 3 brewer or distributor to a designated site for a specific event. (235 ILCS 5/1-3.43 of the Liquor Control Act)A beer showcase permit license shall allow an Illinois-licensed distributor to transfer a portion of its beer inventory from its licensed premises to the premises specified in the beer showcase permit license, and, in the case of a class 3 brewer, transfer only beer the class 3 brewer manufactures from its licensed premises to the premises specified in the beer showcase permit license; and to sell or offer for sale at retail, only in the premises specified in the beer showcase permit license, the transferred or delivered beer for on or off premise consumption, but not for resale in any form and to sell to non-licensees not more than 96 fluid ounces of beer per person. A beer showcase permit license may be granted for the following time periods: one day or less; or 2 or more days to a maximum of 15 days per location in any 12-month period. An applicant for a beer showcase permit license must also submit with the application proof satisfactory to the State Commission that the applicant will provide dram shop liability insurance to the maximum limits and have local authority approval. The State Commission shall require the beer showcase applicant to comply with Section 6-27.1.
