The Business or Organization nominee must be based in the state of Illinois; Nominee must currently have or previously had provided a service or services to one or more veterans. Nominee must be able to provide a record of success with assisting/helping veterans from the calendar year . Elected Illinois officials with businesses are not eligible for this award.
Not-For-Profit Organizations that are nominated must be a certified 501 C3 recognized by the State of Illinois. ALL NFPO that are nominated must complete a state of Illinois verification form which will confirm their 501 C3 status. Honoree(s) will be recognized for significant service to veterans, their contributions made toward local veteran service organizations, as well as state and/or national levels; and by excellence achieved through action above and beyond the call of duty. Nominees must have made important contributions in volunteer work and support of veterans and /or the military.
Please provide a written summary of the nominee’s achievements, including: (a) a description of significant accomplishments, including dates; (b) information concerning any veteran programs they assist; and, (c) nominees business website. In order to validate these accomplishments, please include or attach documentation(s) that will substantiate the business or organization’s service and accomplishments (i.e., news articles, relevant online biographies and/or copies of commendations, etc.).
Nominations will be evaluated on the scope and impact of a nominee’s outreach and/or achievements to the extent that contributions benefit and provide support to veterans in the community. Please document all significant efforts and accomplishments of veterans, especially those efforts that made a significant difference in the veteran’s way of life.
Nominations will be reviewed and the business or organization will be selected by the Illinois Department of Veterans’ Affairs.
Nominations deadline is September 29 for the fiscal year First Quarter (July, August, September); January 15, for the fiscal year Second Quarter( October, November, December) ; April 15,for the fiscal year Third Quarter (January, February, March); July 15, for the fiscal year Fourth Quarter (April, May, June).
The nominee must reside in Illinois and must have received an honorable discharge from the U.S. Military
Posthumous nominations will be accepted provided that a family member of the deceased nominee signs under Signature of Nominee on the Veterans Patriotic Volunteer and Appreciation Award. Honorees will be recognized for significant volunteer service, contributions made at the local, state or national levels or by excellence achieved through action above and beyond the call of duty. Nominees must have made significant contributions in volunteer work in support of veterans and/ or the military.
The nominator needs to provide a legible and up-to-date copy of the veterans DD214. Failure to provide record of service documentation will render the nomination ineligible.
In addition to the DD214 and the information requested on the nomination form, please provide a written summary of the nominee’s achievements, including: (a) a description of significant accomplishments, including dates; (b) information concerning military service; and, (c) a list of military awards and decorations received by the nominee. In order to validate these accomplishments, please include or attach documentation(s) that will substantiate the nominee’s service and the accomplishments (i.e., news articles, relevant online biographies and/ or copies of commendations, etc.).
The nominator must also provide a description of their association and/or, relationship to the nominee.
Nominations will be evaluated on the scope and impact of a nominees achievements and the extent to which his/her efforts benefit and provide inspiration to their community and other veterans. Please document significant achievements and accomplishments, especially those made after the nominee’s military service.
