Re: HFS CARES Program Reporting Template
Available
This
provider notice is being issued to alert subrecipients of the Illinois
Department of Healthcare and Family Services’ (HFS) CARES Program supported by
the Department of the Treasury Coronavirus Relief Fund (CRF) that the final
reporting template is available.
As
stated in the provider notice dated January
15, 2021, subrecipients have until March 29, 2021 to submit the final
reporting template and the supporting documentation.
All
unspent subaward funds must be returned to the Department at the following
address: Receipt Accounting at Illinois Department of Healthcare and
Family Services,
Bureau
of Fiscal Operations, 2200 Churchill Road A-2, Springfield, Illinois 62702.

CARES Final Reporting Template
Post-award
reporting will occur through the HFS CARES Portal. Additional details on the
submission of your final report, the final reporting template and the template
for the required cover letter can be found at the links below:
Round 1
Subrecipients
HFS
CARES Portal accounts were created for subrecipients who received a Round 1
payment but did not register through the HFS CARES Portal. Accounts were
created using the email address used by subrecipients to submit their executed
subaward agreement. Notification emails will be sent to these email addresses
from the HFS CARES Portal, which will include instructions on how to complete
the account registration and set your account password. Once completed,
you will be able to modify or change the primary contact information and
contact email, if necessary.
Round 1
subrecipients who have not received an email notification from the HFS CARES
Portal or need assistance to identify the email address associated with their
account should contact HFS for assistance. Call-in assistance is available from
7 a.m. to 5 p.m. CST, Monday through Friday at 866-385-0600. Call center
support is available in English and Spanish. Providers may also send questions
regarding the CARES payments via email to HFS.CARES@illinois.gov.
The CURE
ID of your subaward will be needed to allow our team to identify the email
address associated with your account. The CURE ID is the Unique Identifier
found on the first page of your subaward agreement.
Round 1
subrecipients whose accounts require updates will still be able to access the
final reporting template and required cover letter template through the links
identified above and may begin preparing their report and gathering the
required supporting documentation.
