Managed Care Community Networks

A Managed Care Community Network (MCCN) is an entity, other than a Health Maintenance Organization (HMO), that is owned, operated, or governed by providers of health care services within Illinois and that provides or arranges primary, secondary and tertiary managed health care services under contract with the Department exclusively to persons participating in programs administered by the Department. The Department contracts with MCCNs, as well as other entities including HMOs, Accountable Care Entities and Care Coordination Entities, as part of its initiative to coordinate the care of Medicaid clients and move at least 50% of all clients into a coordination care system. The MCCN contract for care coordination is very similar to the HMO contracts, the only variance being the financial requirements of the MCCN.
